Ectopic bone formation induced by mechanical stress

Ectopic bone formation (EBF) is frequently found in various tissues and affects the prognosis of diseases accompanied by EBF. Although the mechanism of EBF remains unclear, several local factors influencing the progression of EBF has been proposed. We have been focusing on the role of mechanical stress as a local factor in EBF of spinal ligament tissues, i.e., ossification of the posterior longitudinal ligament (OPLL). Comprehensive analyses such as DNA-microarray and differential display RT-PCR have revealed that expressions of several marker genes related to bone formation are enhanced by exposure of ligament cells from OPLL patients to cyclic stretch as a sort of mechanical stress. However, such alteration of gene expressions by cyclic stretch are not observed in ligament cells from non-OPLL patients. These results suggest that ligament cells from OPLL patients have been transformed to be highly sensitive to mechanical stress which induces progression of OPLL. On the basis of these observations, we will discuss a key role of mechanical stress in ectopic bone formation.
